I acknowledge and agree as follows:
A. By submitting information to LifeSafer on this website, I am requesting to participate in the LifeSafer Partner program (“Program”) and have my name or the name of my company, law firm, or facility (collectively, “Facility”) published in the LifeSafer Partner Directory. I further understand and agree that my eligibility to participate in the Program will be determined solely by LifeSafer and that I may be removed from the Program and de-listed from the Partner Directory at any time, at the sole discretion of LifeSafer. Further, I understand that LifeSafer may choose, at its sole discretion, to end the Program at any time, in whole or in part.
B. As a LifeSafer Partner, I grant LifeSafer permission to publish my or my Facility’s information in the LifeSafer Partner Directory on www.LifeSafer.com.
C. If I am an attorney, I certify that participating in this program does not violate the laws or regulations of any governmental agency or state bar association responsible for the regulation of attorneys and/or attorney advertising in my state.
D. If I am alcohol or other drug (“AOD”) or DUI diversion treatment center or program, I certify that participating in the Program does not violate the laws or regulations of the state(s) in which I or my Facility operates. I further understand and agree that, by participating in the Program, LifeSafer and its affiliates will have no ability to refer clients to me or my Facility, and nothing in the LifeSafer Partner program is designed or intended to result in such referrals. Thus, to the best of my knowledge, becoming a LifeSafer Partner will not violate any provision of the Eliminating Kickbacks in Recovery Act of 2018 (“EKRA”).
E. I understand and agree that nothing in the Program is intended to result in any financial compensation, whether cash or in-kind, or referrals of clients between you or your Facility and LifeSafer.
If you wish to remove your information from the LifeSafer Partner Directory at any time, please send your request to marketing@lifesafer.com.
