Coordinate a Mechanic Repair

If your ignition interlock-equipped vehicle needs to be serviced by a mechanic, LifeSafer is here to help. Each state has specific requirements to allow an authorized mechanic to perform work on your vehicle. Only licensed mechanics and licensed service centers are able to bypass an interlock device.

Any and all actions performed by the mechanic are the responsibility of the vehicle owner. If a mechanic fails to follow instructions to correctly bypass the interlock device and, as a result, causes damage to the unit and/or violations, the LifeSafer client is responsible.

After the service, the mechanic is responsible for returning the car to the original condition or risks violating state regulations and may be subject to penalties.

Step 1: Download the Mechanic Affidavit Form and provide it your mechanic to complete and sign

**You can access and download the mechanic affidavit form below under the “All Other States” section. If you live in Oklahoma, Pennsylvania, Oregon or Washington, you will need to download the state-specific form for your state.

Step 2: Submit the signed and completed mechanic affidavit; and the mechanic’s official work order and receipts via one of the options below.
